Key Responsibilities

• Conduct thorough cardiovascular assessments, including detailed history taking and comprehensive physical examinations to evaluate patients’ health status.
• Educate patients about their heart conditions, helping them understand their diagnosis, treatment options, medication management, lifestyle modifications, and preventive health measures.
• Monitor and document patient progress methodically, with the ability to pivot treatment plans based on ongoing evaluations and patient feedback.
• Participate actively in both inpatient and outpatient care, conducting follow-up appointments to ensure continuity of care and providing discharge planning support for patients transitioning from hospital to home.
• Stay abreast of the latest advancements in cardiology by engaging in continuous learning and applying new knowledge to enhance the quality of care provided to patients.

Patient Population / Client Focus

You will mainly work with adult patients experiencing various heart-related conditions such as heart failure, coronary artery disease, arrhythmias, and hypertension. Your compassionate approach will help build strong relationships with patients, ensuring they feel supported and informed throughout their treatment journey. Engaging with their families to facilitate understanding and compliance with treatment will also be an integral part of your role.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Valid licensure as a Nurse Practitioner in the state of New York.
• A minimum of two years of direct experience in cardiology or a closely related field, demonstrating clinical proficiency and the ability to thrive in a fast-paced healthcare setting.
• Exceptional clinical skills and a profound understanding of cardiovascular medicine.

Preferred Experience

• Experience in patient education and community outreach related to cardiovascular health.

Compensation and Benefits
