Work Environment

Key Responsibilities

• Conduct thorough patient assessments through initial screenings, history taking, and physical examinations, ensuring accurate and insightful evaluations of their health status.
• Maintain detailed and accurate patient records, ensuring compliance with established protocols and regulatory standards at every appointment.
• Examine and interpret diagnostic test results, making informed decisions about treatment plans and follow-up appointments to promote patient wellness.
• Deliver high-quality, evidence-based care, consistently aligning with productivity metrics and quality assurance benchmarks set forth by the organization.
• Act as a trusted resource for patients regarding ongoing care, providing counseling, health resources, and guidance tailored to those with complex medical conditions.

Patient Population / Client Focus

The Family Nurse Practitioner will cater to a diverse patient population, including individuals with acute and chronic health conditions across various age groups. Qualifications

• Cerfitication as a Family Nurse Practitioner (FNP) within the Family Medicine specialty.
• Possession of a valid and active FNP license in the state of New York.
• A Master's degree in nursing, completed from an accredited program focused on acute/primary health care services.
• A minimum of two years of clinical experience in a Family Nurse Practitioner role or a related specialty.
• Proficiency in performing comprehensive physical examinations and diagnosing common health issues, including managing ongoing chronic conditions.

Preferred Experience

• Experience within a home care setting or a similar community-based environment is preferred, enhancing familiarity with the complexities of home health management.

Compensation and Benefits

This position offers a competitive annual salary in the range of USD 150,000 to 160,000, commensurate with experience and qualifications.
