Key Responsibilities

• Conduct comprehensive assessments to diagnose and manage acute and chronic health conditions of patients in a home care environment.
• Prescribe necessary medications and develop tailored treatment plans based on each patient's individual health status and needs.
• Perform detailed physical examinations, ensuring alignment with specific care protocols and best practices.
• Implement patient management policies, ensuring that each patient receives the highest standard of care while ensuring compliance with regulations.
• Coordinate with physicians, specialists, and additional healthcare providers to facilitate seamless referrals and continuity of care.
• Engage actively in patient education regarding health maintenance, disease prevention, and lifestyle modifications that enhance overall wellness.

Patient Population / Client Focus

In this role, you will cater to a diverse clientele, including infants, elderly individuals, and those with chronic illnesses, adapting your care approach to meet the unique requirements of each patient. Qualifications

• A valid Nurse Practitioner license in New York State, with a Bachelor's Degree in Nursing or a related field.

Compensation and Benefits

The compensation for this position ranges from USD 150,000 to 160,000 yearly , which reflects our commitment to recognizing the value of experienced Nurse Practitioners.
