Adult-Gerontology Nurse Practitioner - Full-Time Home Care Position

Position Overview

Riverhead Physical Medicine and Rehabilitation is seeking dedicated and enthusiastic Adult-Gerontology Nurse Practitioners to join our dynamic team in Tonawanda, New York. This full-time, onsite position focuses on delivering compassionate and value-based primary care to adults, particularly those on Medicare, with an annual salary ranging from USD 120,000 to 160,000.

Work Environment

The Nurse Practitioner will work primarily in a home care setting, collaborating with a cohesive team composed of physicians, physician assistants, nurse practitioners, and support staff including medical assistants and registered nurses. Our operational framework is designed to facilitate interdisciplinary collaboration, ensuring optimal patient care. This role allows for deep engagement with our patient population, as our practitioners not only conduct in-clinic visits but also perform home visits as necessary, providing a comprehensive care experience.

Key Responsibilities

• Deliver exceptional primary care through in-office evaluations, focusing on both routine and acute healthcare needs.
• Conduct annual wellness visits and health risk assessments, ensuring thorough and precise documentation of each patient's health status.
• Coordinate patient care by collaborating effectively with specialists, testing facilities, and ancillary health services, providing seamless transitions and comprehensive treatment plans.
• Lead initiatives in population health management, such as ensuring eligible patients receive routine screenings and preventive services tailored to their health profiles.
• Participate in team-based phone triage and patient outreach, addressing healthcare queries and enhancing service delivery.
• Educate patients regarding their chronic conditions, care plans, and treatment options, fostering understanding and compliance.
• Promote the organization’s services and participate in community outreach initiatives aimed at raising awareness about our patient-centric approach.
• Conduct home visits as needed to assess patients’ health conditions in their own environments, providing hands-on comprehensive care.
• Countrywide reporting to the Medical Director while closely collaborating with operational leadership to drive quality improvements.
• Perform other duties as assigned by senior management.

Patient Population / Client Focus

This role focuses primarily on older adults living in the Tonawanda area, many of whom are Medicare beneficiaries with diverse health needs. By understanding and responding to the unique challenges faced by geriatric populations, our Nurse Practitioners play a pivotal role in enhancing patient outcomes and ensuring effective chronic disease management.

Qualifications

• A Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or successful completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized accrediting bodies such as AANP or ANCC.
• Possession of a current, active, and non-probationary Nurse Practitioner license in New York State.
• Active DEA license necessary for prescribing medications.
• Demonstrated experience in primary care, particularly in internal medicine, geriatrics, or family medicine settings.
• Proficient understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding practices, and health risk assessments.
• A strong team player with a collaborative spirit and a commitment to continuous improvement in patient care practices.

Preferred Experience

• A background in utilizing electronic health record (EHR) systems to streamline documentation and patient care coordination.
• Familiarity with community resources and referral processes, enhancing patient access to ancillary services.
• Experience in leading educational initiatives or health promotion activities focused on senior health.
