Work Environment

This role is based onsite within a hospital facility, offering a collaborative practice environment. As part of our dynamic care team, you will work alongside physicians, other nurse practitioners, physician assistants, medical assistants, and skilled registered nurses. We also have a dedicated medical scribe to assist with documentation, allowing you to concentrate on patient care. Our collaborative approach ensures that healthcare providers can focus on primary care, care coordination, and patient-centered population health initiatives.

Key Responsibilities

• Deliver exceptional primary care to adult patients, focusing on individualized treatment plans.
• Conduct thorough office visits to address both routine and acute medical issues.
• Administer annual wellness visits and perform comprehensive health risk assessments, ensuring meticulous documentation for optimal patient care.
• Coordinate care with a network of specialists and support services to facilitate comprehensive patient management.
• Lead initiatives in population health, such as ensuring eligible patients receive timely screenings and preventive care services.
• Assist the care team with phone triage, patient outreach, and follow-up communications.
• Educate patients on their health conditions, treatment options, and self-management techniques to promote proactive health behaviors.
• Participate in community outreach and promotional activities to enhance our care model and engage with the local population.
• Conduct home visits when necessary to support patients who may have mobility challenges or require in-home assessments.
• Report directly to the Medical Director and work closely with operational leadership for seamless clinic operations.

Patient Population / Client Focus

Your primary focus will be on Medicare-eligible adults, especially older adults who require nuanced care strategies to manage chronic conditions and comorbidities effectively. The aim is to keep this population healthy while promoting independence and active living. Our primary care model is developed around the principle of proactive care, striving to maintain patient wellness and prevent unnecessary hospital admissions.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or an equivalent Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized bodies such as the AANP or ANCC.
• An active, non-probationary Nurse Practitioner license in New York State.
• A current DEA license for prescribing medications.
• Demonstrable experience in primary care, geriatrics, or family medicine.
• Knowledge of Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and Health Risk Assessments is essential.
• Strong collaborative spirit and passion for teamwork in a multidisciplinary setting.

Preferred Experience

• Previous experience in a value-based care model is advantageous.
• Proficiency in utilizing electronic health records (EHR) systems for documentation and patient management.
• Experience in conducting community health education or outreach activities.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 120,000 to 160,000 per year, commensurate with experience.
