Work Environment

This position will require the Nurse Practitioner to engage directly with patients in various settings within Tonawanda, New York, primarily focusing on house calls and concierge services. You will work collaboratively with a multidisciplinary team of healthcare professionals, including attending consultants, to ensure comprehensive care delivery. The ideal candidate will possess excellent organizational skills and demonstrate the ability to independently manage a busy caseload while maintaining high standards of patient care.

Key Responsibilities

• Order and interpret diagnostic tests to guide patient care decisions effectively.
• Identify and manage both acute and chronic health conditions according to established clinical guidelines.
• Develop and implement individualized care plans that may include medication management, nutritional counseling, and lifestyle modifications.
• Collaborate with healthcare providers to coordinate patient care across different disciplines and make referrals as necessary.
• Monitor and document patient progress, adjusting care plans based on individual responses.
• Engage in telephone consultations, follow-up on laboratory results, and assist walk-in patients with triage and referrals.
• Perform thorough health histories and targeted physical exams based on patient concerns and needs.
• Advise and mentor non-medical staff and newly hired nurse practitioners, sharing clinical expertise and methods.
• Participate in the development and refinement of practice guidelines to enhance agency operations.
• Conduct audits of peer practitioners’ work to uphold quality care standards and participate in quality assurance initiatives.
• Ensure confidentiality of patient information and demonstrate sensitivity in all interactions.
• Provide direct patient care in clinical sessions, ensuring engaging and responsive services.

Patient Population / Client Focus

You will primarily work with a diverse group of patients, providing care tailored to their unique medical histories and needs. This role places a strong emphasis on holistic approaches to health management, prioritizing long-term wellness and chronic disease management as seen in the concierge medicine model. By connecting closely with patients, you will engage in meaningful discussions that empower them to take an active role in their health.

Qualifications

• Active New York State Nurse Practitioner license and certification in either Family Health or Adult Health.
• A preferred NYS Registered Nurse license is a plus.
• Possession of a current NPI number and DEA certificate.
• A Master’s Degree in Nursing is required for this position.

Preferred Experience

• A minimum of three years of relevant experience, particularly in ambulatory care or concierge settings.
• Demonstrated ability to work collaboratively in a healthcare team.
• Proficient in electronic health record systems and telemedicine platforms.

Compensation and Benefits

The salary for this role ranges from USD 150,000 to 160,000 annually, reflecting our commitment to recognizing the expertise and dedication of our NPs.
