The Evergreen Family Practice is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our team in Buffalo, New York. Key Responsibilities

• Lead clinic staff, including X-Ray Technologists, Nurses, and Front Office Specialists, to deliver consistent, high-quality patient care.
• Provide clinical guidance and mentorship to colleagues to promote best practices and ensure patient satisfaction.
• Accurately complete and review patient charts on a daily basis, ensuring timely documentation and compliance with organizational protocols.
• Implement clinical protocols as defined by the Chief Medical Officer and supervising physicians to enhance service delivery.
• Maintain compliance with federal and state health regulations, including HIPAA, to safeguard patient information.
• Order, perform, or interpret diagnostic tests and prescribe medications responsibly, thoroughly educating patients on their treatment options and management of conditions.
• Perform a variety of therapeutic procedures, including incision and drainage, splinting, suturing, and managing infections or wounds.
• Analyze patients' medical histories, symptoms, and diagnostic information to develop accurate diagnoses and treatment plans.

• Complete additional tasks as assigned by management to aid in achieving organizational goals.

Patient Population / Client Focus

You will work with a diverse patient population, ranging from children to adults with acute and chronic health conditions, including but not limited to those requiring ongoing home care management. By focusing on individual needs and fostering a supportive atmosphere, you will play a crucial role in educating patients about their health, enhancing their engagement and adherence to treatment plans.

Qualifications

• A Master's Degree in Nursing with a focus on Family Nurse Practitioner training.
• Must possess a valid Family Nurse Practitioner License to practice in New York.
• Demonstrated ability to adhere to company operational standards and healthcare regulations.
• Strong interpersonal skills and the ability to maintain positive relationships with clients and teammates.
• Experience consulting with patients via telehealth platforms is highly advantageous.

Preferred Experience

• At least 2 years of experience as a Nurse Practitioner, preferably in a home care or telehealth setting.
• Proven ability to work effectively in a fast-paced and self-directed environment.
• Experience in patient triage and chronic disease management.
