Work Environment

Your role will be onsite, allowing for direct interactions with both patients and an interdisciplinary team of healthcare providers. The work schedule typically ranges from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. You may also engage in community outreach events and health fairs during weekends to further our mission of promoting health awareness and education.

Key Responsibilities

• Perform comprehensive health assessments and screenings tailored to the needs of individuals and families, ensuring a holistic approach to healthcare delivery.
• Diagnose and manage both acute and chronic medical conditions, prioritizing preventive care and health promotion strategies to enhance patient well-being.
• Collaborate with community organizations, healthcare systems, and stakeholders to design and implement effective health education programs aimed at empowering community members.
• Provide culturally sensitive care, advocating for patients' rights and ensuring equitable access to healthcare services across diverse populations.
• Coordinate referrals and follow-up care meticulously to maintain continuity of care, addressing any challenges the patient might encounter along their healthcare journey.
• Maintain comprehensive and accurate medical records in alignment with regulatory standards, utilizing electronic health record systems for efficiency and compliance.
• Engage in community outreach activities, including health fairs, to educate the public and conduct health screenings to reach vulnerable populations.

Patient Population / Client Focus

You will primarily serve a diverse patient population, including children, adults, and families within the Buffalo area. You will be expected to approach each patient holistically, tailoring your care to meet their cultural, emotional, and physical needs.

Qualifications

• Current certification as a Nurse Practitioner is essential.
• An active state nursing license is required for practice.
• A Master’s degree in nursing from an accredited institution is mandatory.

Compensation and Benefits

• We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
