Work Environment

This full-time position is conducted onsite at a school facility, allowing you to engage directly with children and their families in a familiar setting. Your typical work hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. You may also engage in community outreach activities, including health fairs and weekend events, giving you opportunities to better serve the community and promote health education.

Key Responsibilities

• Conduct thorough health assessments and screenings for pediatric patients and their families, identifying health concerns and establishing care plans.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health promotion strategies tailored to each patient’s needs.
• Collaborate actively with community organizations, schools, and stakeholders to implement health education programs that promote wellness and disease prevention.
• Provide culturally sensitive care that respects diverse backgrounds, advocating for patients' rights and access to necessary healthcare services.
• Coordinate referrals and follow-up care to ensure consistent and accessible healthcare for all patients.
• Maintain comprehensive medical records in compliance with state regulations, ensuring accurate documentation of patient care and progress.
• Engage in community outreach to raise awareness regarding health issues, providing screenings, and health education.
• Stay informed of current trends in community health, practicing evidence-based strategies to continuously improve care delivery.

Patient Population / Client Focus

Your primary focus will be serving pediatric patients and their families within the Buffalo community, many of whom come from underserved populations. This role requires a commitment to enhancing healthcare access and outcomes for these demographics, acknowledging cultural sensitivities and unique health concerns that may arise.

Qualifications

• Current certification as a Nurse Practitioner.
• An active nursing license in the state of New York.
• A Bachelor’s degree in Nursing, with a strong preference for candidates holding a Master’s degree in Nursing from an accredited institution.

Preferred Experience

We seek candidates with 3-5 years of clinical experience, particularly in community health or public health settings. Experience dealing with pediatric populations and familiarity with various health education methodologies is highly valued. Strong interpersonal and communication skills are essential for effective collaboration within a diverse community.

Compensation and Benefits

The salary for this position ranges from $120,000 to $220,000 per year, reflective of your experience and qualifications.
