Work Environment

This position will be based onsite at an Assisted Living Facility in Tonawanda, where you will be part of a collaborative care team consisting of a physician, other nurse practitioners or physician assistants, medical assistants, and care managers. The facility is equipped with the latest medical technologies and resources, allowing you to deliver well-coordinated and efficient care.

Key Responsibilities

• Provide exceptional primary care to patients during routine and acute office visits.
• Conduct comprehensive Annual Wellness Visits and health risk assessments that require holistic evaluations and detailed documentation.
• Coordinate care with other healthcare providers, specialists, and testing facilities to ensure an integrated healthcare experience for patients.
• Lead population health initiatives, such as ensuring eligible patients receive necessary screenings, like bi-annual mammograms.
• Assist the care team with phone triage, patient outreach, and follow-up.
• Educate patients about their health conditions, care plans, treatments, and preventive health measures.
• Participate in promotional activities for the clinic and community health initiatives.
• Conduct home visits as necessary to provide personalized patient care.
• Report to the Medical Director while maintaining close collaboration with operational leadership.

Patient Population / Client Focus

Your primary focus will be on serving the older adult population, particularly those on Medicare, ensuring they maintain optimal health and quality of life. Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from recognized bodies such as AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and DEA license.
• Experience in primary care settings, specifically within internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Preferred Experience

• A demonstrated passion for teamwork, with readiness to collaborate cross-functionally.

• Bilingual proficiency is a plus, enhancing communication with diverse patient populations.

Compensation and Benefits

The position offers a competitive salary ranging from USD 115,000 to USD 150,000 annually, with the final salary depending on the candidate's experience and expertise level. In addition, we provide a comprehensive benefits package that includes:
