Full-Time Nurse Practitioner for Internal Medicine at State Health, LLC

Position Overview

We are seeking a dedicated and compassionate Nurse Practitioner specializing in Internal Medicine to join our proactive healthcare team at State Health, LLC. This full-time position in Niagara Falls, New York, focuses on providing exceptional primary care to Medicare-eligible adults in an Assisted Living Facility setting. As a Nurse Practitioner, you will work alongside a collaborative care team to enhance patient outcomes and deliver high-quality, equitable healthcare.

Work Environment

The successful candidate will work onsite at our Assisted Living Facility located in the beautiful Niagara Falls area. You will also have the advantage of a dedicated medical scribe, a supportive team of medical assistants, and care managers who work together to streamline patient care. Key Responsibilities

• Deliver high-quality primary care services to adult and geriatric patients by conducting comprehensive office visits for both routine and acute medical issues.
• Perform annual wellness visits and health risk assessments, providing a holistic view of patients' health and ensuring precise medical documentation.
• Engage in care coordination with other healthcare providers, specialists, and community resources to optimize patient pathways and outcomes.
• Lead initiatives in population health management, including overseeing compliance with preventive care measures such as breast cancer screening every two years for eligible female patients.
• Contribute to patient education, outlining care plans, health conditions, and treatments to empower patients in their health management.
• Participate in promotional efforts to increase community awareness and engagement.
• Conduct home visits when necessary to ensure continuity of care, especially for those patients unable to visit the facility.
• Collaborate closely with leadership and report to the Medical Director on clinical pathways, operational improvements, and patient satisfaction metrics.

Patient Population / Client Focus

The primary focus of this role will be on older adults who are typically on Medicare, many of whom may have chronic health conditions requiring ongoing management. You will be providing personalized and empathetic care for a diverse patient population, recognizing the unique healthcare needs of seniors. Qualifications

• A Master of Science in Nursing, a Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through either the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license is required to practice in New York, along with an active DEA license.
• Experience in primary care with a focus on internal medicine, geriatrics, or family medicine is essential.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is required.
• A demonstrated passion for teamwork and a commitment to collaborative, cross-functional care delivery.
• Bilingual proficiency is a plus, enhancing communication with our diverse patient base.

Preferred Experience

Prior experience in a primary care setting, particularly in an assisted living or community health environment, is strongly preferred. Familiarity with innovative care models focused on population health will be beneficial.

Compensation and Benefits

The salary range for this position is between USD 115,000 and 150,000 per year, with the potential for a wider range depending on experience and expertise. Our benefits package includes:
