This position is based at a school facility located in the vibrant community of Tonawanda, New York. Key Responsibilities

• Conduct thorough patient histories and physical examinations to assess individual health conditions.
• Develop, document, and communicate individualized care plans based on patient assessment and collaborative discussions with the healthcare team.
• Perform daily patient rounds to monitor health status, provide care, and adjust treatment plans as necessary.
• Prescribe medications, request laboratory tests, and order diagnostic imaging when required to ensure comprehensive care.
• Analyze laboratory and diagnostic results, correctly interpreting findings to inform ongoing patient management.
• Collaborate with attending physicians, nursing staff, and other healthcare providers to create an integrated approach to patient care.
• Prepare detailed discharge summaries, including prescriptions and referrals, ensuring patients understand follow-up instructions.
• Engage in both performance improvement and educational initiatives, contributing to research projects as relevant.
• Perform other related responsibilities as needed, ensuring that all activities adhere to relevant regulations and standards set forth by the organization and industry.

Patient Population / Client Focus

The primary patient population for this position includes a diverse group of children and adolescents attending the school, facing various health issues that require acute care as well as ongoing management of chronic conditions. As a Nurse Practitioner, you will play a vital role in promoting wellness, preventive care, and health education within this community environment, addressing both physical and emotional aspects of health.

Qualifications

• A Master's Degree in a Nurse Practitioner Program is required, along with National Board Certification in the specialty area.
• Candidates must hold an active Nurse Practitioner license and a Registered Professional Nurse license in New York State.
• A collaborative practice agreement with designated protocols must be filed with the NYS Department of Education, or an attestation of a collaborative relationship as mandated by the Nurse Practitioner Modernization Act (2015).
• Current Basic Life Support (BLS) certification is mandatory for all applicants.

Preferred Experience

• Experience working as a Nurse Practitioner in a school or pediatric setting is preferred, as this will enhance your ability to address the specific needs of younger populations.
• Previous involvement in performance improvement initiatives and research projects is advantageous.

Compensation and Benefits

Northwell Health offers a competitive salary range of USD 150,000 – 160,000 annually, commensurate with experience and qualifications.
