Key Responsibilities

• Conduct thorough patient assessments by performing comprehensive medical, surgical, and psychosocial histories, and document findings accurately in electronic health records.
• Order, interpret, and discuss diagnostic studies such as laboratory tests and imaging studies, making informed diagnostic impressions based on data collected.
• Develop and implement personalized care plans in collaboration with other healthcare professionals, ensuring compliance with institutional policies and regulatory requirements.
• Provide education and counseling to patients and their families on health management, including disease prevention, nutrition, and mental health strategies.
• Maintain up-to-date knowledge of best practices and participate in quality improvement initiatives to enhance the care delivered.
• Actively engage with a multidisciplinary care team to participate in the planning and provision of seamless treatment across all settings.

Qualifications

• Board certification in Family Medicine or Internal Medicine.
• Graduation from an accredited Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program.
• An active, unrestricted state license to practice as a Nurse Practitioner in New York.
• Authorization to work in the United States.

Compensation and Benefits

The compensation for this position offers a competitive annual salary range of $150,000 to $160,000, depending on experience and caseload management.
