Work Environment

This role is entirely onsite, where you will conduct house calls to deliver healthcare directly to patients' homes. This setting fosters an intimate atmosphere that allows for deeper patient interactions and assessments. Key Responsibilities

• Collaborate closely with physicians and a multidisciplinary team to design comprehensive healthcare plans tailored for individual patients.
• Conduct thorough patient assessments, obtaining detailed medical histories and performing age-appropriate physical examinations.
• Implement preventive screening procedures to identify potential health issues early, based on the patient's age and medical history.
• Formulate differential diagnoses, utilizing advanced clinical judgment grounded in your assessments and available data.
• Develop and execute personalized education plans that empower patients in understanding their health conditions and treatment options.
• Prescribe both pharmacologic and non-pharmacologic therapies, adjusting treatment plans based on ongoing assessments and patient feedback.
• Maintain accurate and comprehensive electronic health records (EHR), utilizing technology to provide timely updates and continuity of care.

Patient Population / Client Focus

You will cater to a diverse group of patients, from young adults to seniors, all with varying degrees of medical complexities. Your focus will be on providing continuous care that emphasizes preventative measures, chronic illness management, and holistic health approaches. Qualifications

• A Master’s Degree in Nursing with a specialization as a Nurse Practitioner is required.
• An active Registered Nurse (RN) license in the State of New York.
• A minimum of 3-4 years of relevant clinical experience, demonstrating your ability to manage diverse medical conditions effectively.
• Familiarity with electronic health record (EHR) systems and proficiency in using medical technology essential for patient care.
• Strong interpersonal skills with a commitment to educating patients and advocating for their health needs.

Preferred Experience

Compensation and Benefits

We offer a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience. In addition to a robust salary, our benefits package includes:
