Community Health Nurse Practitioner - Full-Time, Onsite in East Amherst, NY

Position Overview

We are seeking a dedicated and compassionate Community Health Nurse Practitioner to join our team at the US Veterans Health Care Clinic, located in East Amherst, New York. This full-time position offers an opportunity to deliver holistic healthcare to individuals and families in our community from Monday to Friday, during standard business hours of 9 AM to 5 PM. Key Responsibilities

• Develop and implement health education programs tailored to meet the diverse needs of the community, utilizing evidence-based practices and the latest clinical guidelines.
• Provide culturally sensitive and individualized care, advocating for improved access to healthcare services for all patients, especially those from underserved backgrounds.
• Maintain precise medical records and documentation in accordance with local and federal regulations, ensuring confidentiality and compliance with best practices.
• Participate in community outreach initiatives, including health fairs, workshops, and screening events, to promote awareness and support preventative health measures.
• Stay informed of current trends in community health, participate in professional development opportunities, and collaborate with healthcare teams to enhance service delivery.

Patient Population / Client Focus

This position primarily focuses on serving veterans and their families residing in the assisted living facility, as well as surrounding communities. You will encounter a diverse population with varying healthcare needs, ranging from routine health assessments to chronic disease management. A strong emphasis will be placed on developing tailored care plans to address the unique challenges and health issues faced by this population, fostering strong relationships founded on trust and respect.

Qualifications

• A Bachelor's or Master's degree in Nursing from an accredited program is required.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State is essential.
• DEA and NPI numbers are required for prescription management and billing purposes.
• Excellent clinical assessment, diagnostic skills, and the ability to make sound clinical judgements.

Preferred Experience

• While a minimum of 2 years of clinical experience is required, candidates with 3-5 years of experience in community health or public health settings are preferred.
• Demonstrated ability to communicate effectively with diverse populations, tailoring approach to meet individual needs.
• Experience collaborating with community organizations for health initiatives or education.

Compensation and Benefits

This position offers a competitive annual salary in the range of USD 150,000 to 160,000, commensurate with qualifications and experience.
