Work Environment

This role is based at NextGen NP Solutions’ facility in Lockport, New York, where you will primarily engage with patients in their homes. Your typical hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. In addition, you may participate in weekend community outreach events and health fairs, enhancing your engagement with diverse populations and increasing the visibility of community health initiatives.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families in various community settings, employing advanced clinical skills to evaluate health needs.
• Diagnose and manage a range of acute and chronic medical conditions, implementing evidence-based preventive care strategies tailored to individual patient profiles.
• Develop, coordinate, and implement health education programs in partnership with local organizations, focusing on critical health issues relevant to the community.
• Deliver culturally sensitive care while advocating for patients' rights and facilitating access to necessary healthcare services.
• Coordinate timely referrals and follow-up care, ensuring continuity of care, maintaining clear communication among all parties involved.
• Document patient interactions and clinical findings accurately and comprehensively, adhering strictly to regulatory standards and confidentiality protocols.
• Engage in community outreach initiatives and health fairs, providing vital health screenings and raising awareness of public health concerns.
• Stay informed on emerging community health trends, evidence-based practices, and relevant healthcare policies that impact patient care.

Patient Population / Client Focus

As a Community Health Nurse Practitioner, you will work with a diverse population of patients ranging from children to the elderly, many of whom may have limited access to traditional healthcare settings. You will address specific health disparities present in the Lockport community while fostering relationships based on trust and understanding to improve overall health outcomes.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of valid DEA and NPI numbers.
• Strong clinical assessment and diagnostic skills, specifically related to primary care and preventive health.
• Exceptional communication and interpersonal skills, capable of building rapport with patients from diverse backgrounds.
• Experience in utilizing electronic health record (EHR) systems efficiently for documentation and care coordination.

Preferred Experience

• A minimum of 3-5 years of clinical experience, with a preference for backgrounds in community health or public health settings.
• Demonstrated experience in providing culturally competent care and navigating healthcare disparities within various populations.
• A strategic mindset with the ability to adapt to changing community needs and health trends.

Compensation and Benefits

Compensation for this position ranges from USD 150,000 to 160,000 annually. In addition to a competitive salary, NextGen NP Solutions offers a comprehensive benefits package that includes health insurance, paid time off, professional development opportunities, and a sign-on bonus of up to $10,000 for qualified candidates.
