Full-Time Nurse Practitioner for Home Care Services

Position Overview

Advanced Cardiology PC is seeking dedicated and compassionate Nurse Practitioners to join our innovative healthcare team in East Amherst, New York. This full-time position focuses on delivering exceptional primary care to adult patients, particularly those covered by Medicare. With a competitive salary range of USD 120,000 – 160,000 annually, our Nurse Practitioners play a crucial role in enhancing patient health and well-being through personalized healthcare solutions delivered in the comfort of their homes.

Work Environment

This role is primarily onsite, with a strong emphasis on home care services. Nurse Practitioners will work within an interdisciplinary team consisting of physicians, physician assistants, medical assistants, nurse managers, and dedicated medical scribes. Our collaborative environment ensures that Nurse Practitioners have access to significant support, allowing them to focus on their core responsibilities of patient care and health coordination. Our practice utilizes advanced healthcare technology to streamline workflows, improve documentation, and enhance overall patient experiences.

Key Responsibilities

• Deliver comprehensive primary care with a focus on preventive health.
• Conduct routine examinations and evaluate acute health issues during office visits.
• Execute annual wellness visits and perform health risk assessments that require thorough medical histories and tailored documentation.
• Coordinate care liaising with other healthcare providers, specialists, and testing facilities to ensure a holistic approach to patient health.
• Take a leadership role in population health initiatives, ensuring patients receive essential health screenings and preventive services, such as bi-annual breast cancer screenings for eligible women.
• Assist the care team with phone triage and proactive outreach to patients for follow-ups and health checks.
• Educate patients about their health conditions, proposed treatments, and the significance of their care plans through effective communication and support.
• Engage in community outreach and promotional activities that enhance patient awareness and access to healthcare services.
• Conduct home visits for patients unable to attend the clinic due to health constraints.
• Collaborate closely with the Medical Director and operational leadership for reporting and strategic enhancements.

Patient Population / Client Focus

Our patient population primarily consists of older adults on Medicare. We aim to provide personalized, high-quality primary care that adapts to the evolving needs of our patients. The mission is to promote proactive health management, enable patients to lead fulfilling lives, and improve overall health outcomes in our community. Our goal is to reduce hospital admissions through effective management of chronic health conditions and prevention strategies.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active, non-probationary state Nurse Practitioner license.
• Current and valid DEA license.
• Prior experience in primary care settings such as internal medicine, geriatrics, or family medicine.
• Knowledgeable in HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) to ensure compliant and comprehensive patient documentation.
• A strong passion for teamwork and collaboration in a healthcare setting.

Preferred Experience

• Experience working with geriatric patients and understanding the complexities of managing multiple chronic conditions.
• Familiarity with value-based care models and their implementation in a primary care environment.
• Proven track record in driving population health initiatives and improving care coordination.
