Key Responsibilities

• Conduct thorough patient assessments, including initial screenings, history-taking, and physical examinations to establish comprehensive care plans tailored to individual needs.
• Maintain and update accurate patient records to ensure continuity of care. Review records at each appointment to check for accuracy, ensuring compliance with healthcare regulations.
• Interpret and analyze test results, making informed decisions on further treatment options based on evidence-based practices.
• Collaborate with patients to develop personalized care plans, ensuring they understand their health conditions and treatment pathways.
• Educate patients and their families on disease prevention, health maintenance, and safe practices, placing emphasis on minimizing risks and enhancing quality of life.

Patient Population / Client Focus

You will be primarily responsible for a diverse adult patient population, many of whom may have chronic health conditions requiring extended management and support. Qualifications

• A Master's degree in Nursing from an accredited institution.
• A valid and unencumbered New York State Nurse Practitioner (NP) license.
• A minimum of two years experience as a licensed Nurse Practitioner, with a strong background in family medicine or primary care.
• Exceptional communication and interpersonal skills, particularly in handling sensitive medical situations and navigating complex documents.

Preferred Experience

• Experience in a family practice or home care clinic setting is highly desirable.

Compensation and Benefits
