The Elderly Care & Rehab Clinic is seeking a dedicated and compassionate Nurse Practitioner to join our team in East Amherst, New York. Work Environment

This position is based onsite in a home care setting within a supportive clinic environment. Our integrated care team includes physicians, physician assistants, medical assistants, registered nurses, and care managers, all working cohesively to prioritize patient health. The collaborative atmosphere encourages open communication and provides all team members with the tools necessary to deliver outstanding healthcare services.

Key Responsibilities

• Deliver high-quality primary care in a clinical setting, ensuring patient needs are met with empathy and expertise.
• Manage routine and acute patient visits while conducting thorough assessments and developing individualized care plans.
• Administer annual wellness visits and comprehensive health risk assessments, emphasizing the importance of accurate documentation and holistic health strategies.
• Coordinate care with other healthcare providers, specialists, and testing facilities to establish a seamless continuum of care for every patient.
• Lead initiatives in population health management, ensuring all eligible patients receive life-saving screenings and preventative services, such as biennial breast cancer screenings.
• Support the care team through triage, follow-up calls, and proactive patient outreach, improving patient engagement and satisfaction.
• Educate patients about their conditions, explaing treatment options and encouraging adherence to their care plans for improved outcomes.
• Participate in clinic promotional activities and represent the organization positively in the community.
• Conduct home visits when necessary for patients with mobility challenges or other special needs.
• Report directly to the Medical Director while collaborating closely with operational leadership to ensure adherence to best practices in patient care.

Patient Population / Client Focus

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or a graduate from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Current, active, and non-probationary Nurse Practitioner license in New York State.
• DEA license in good standing.
• Demonstrated experience in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and the administration of Health Risk Assessments (HRAs).
• A collaborative mindset with a passion for working within a multidisciplinary team.

Preferred Experience

• Previous experience working with older adult populations in a primary care or rehabilitation setting.
• Expertise in managing chronic health conditions and implementing preventative care strategies.
• A strong understanding of community health resources and how to effectively navigate them for patient benefit.

Compensation and Benefits

This full-time position offers a competitive salary in the range of USD 120,000 – 160,000 annually, commensurate with experience. Additionally, we provide comprehensive benefits packages, including health insurance, retirement plan options, and paid time off to support a healthy work-life balance.
