Work Environment

You will work onsite in our outpatient office, where you will be part of a collaborative healthcare team made up of physicians, physician assistants, medical assistants, and dedicated support staff, including medical scribes, registered nurses, and care managers. This integrative approach ensures seamless care coordination and reinforces our commitment to assisting our patients with their health journeys. The work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, allowing for work-life balance while serving your community.

Key Responsibilities

• Deliver exceptional primary care tailored to each patient's individual needs, addressing both routine and acute health issues.
• Conduct thorough office visits that include assessing patient health, diagnosing conditions, and formulating appropriate treatment plans.
• Administer annual wellness visits and comprehensive health risk assessments, emphasizing accurate documentation to support continuity of care.
• Facilitate care coordination with specialists, diagnostic testing facilities, and community agencies to ensure comprehensive patient management.
• Lead population health initiatives in collaboration with the Care Team, ensuring preventive screenings, such as breast cancer and diabetes assessments, are accessible to all eligible patients.
• Assist fellow care team members with phone triage, outreach programs, and patient follow-ups to enhance patient engagement.
• Educate patients on their health conditions, treatment options, and care plans to empower them in their health management.
• Participate in community outreach and promotional activities to enhance awareness of the services we provide.
• Conduct home visits for patients who are homebound or require additional assistance, ensuring holistic care delivery.
• Report directly to the Medical Director while cooperating closely with operational leadership to improve care delivery processes.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare, who may face chronic illnesses and require thoughtful, personalized care management. By understanding their unique healthcare needs, you will contribute significantly to their quality of life and health independence. Qualifications

• A Master's Degree in Nursing, a Doctor of Nursing Practice, or graduation from an accredited Nurse Practitioner certificate program is mandatory.
• Nationally recognized certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Current DEA license for prescribing medications.
• Previous experience in primary care settings, specializing in internal medicine, geriatrics, or family medicine is highly desirable.
• Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding systems, and Health Risk Assessments (HRAs).
• A genuine passion for teamwork and a collaborative spirit to improve team efficiency and patient care outcomes.

Preferred Experience

• Experience in managing a diverse patient population with multiple chronic conditions, enabling better health outcomes through targeted interventions.

Compensation and Benefits

The salary for this position ranges from USD 120,000 to 160,000 yearly, commensurate with experience.
