Work Environment

This role will primarily take place within an assisted living facility located in North Tonawanda, where you will work closely with both medical and support staff to create a nurturing environment for our residents. The facility is designed to support the daily living needs of our residents while promoting their health and wellness. You will operate within a structured schedule, typically from 9:00 a.m. to 6:00 p.m., with flexibility required for occasional evening clinics and participation in weekend community outreach events.

Key Responsibilities

• Perform thorough health assessments and screenings, gathering detailed medical histories and performing physical examinations for residents of the assisted living facility.
• Develop and implement individualized care plans, addressing both acute and chronic health conditions, including preventive care measures and health promotion strategies.
• Collaborate with other healthcare professionals and community organizations to initiate and execute effective health education programs aimed at enhancing resident health literacy.
• Maintain meticulous and compliant medical records, ensuring accuracy and confidentiality in accordance with regulatory standards.
• Advocate for patients' rights, fostering an inclusive healthcare environment that prioritizes culturally sensitive care.
• Coordinate referrals to specialists and follow-up care, ensuring seamless transitions and continuity of healthcare for residents.
• Participate actively in community outreach activities, health fairs, and wellness events to increase community awareness and access to healthcare resources.

Patient Population / Client Focus

In this role, you will primarily serve the elderly population residing in the assisted living facility. Many residents may face complex health challenges requiring comprehensive management of multiple chronic conditions. Qualifications

• Master's degree in Nursing from an accredited program, with a current certification as a Nurse Practitioner.
• Active state nursing license in New York.
• Minimum of 4 years of relevant clinical experience; experience in community health, geriatrics, or assisted living settings is highly valued.
• Exceptional interpersonal and communication skills to effectively engage with diverse populations, developing strong therapeutic relationships with patients.
• A strong commitment to health equity, social justice, and advocacy for vulnerable communities.

Preferred Experience

Compensation and Benefits

This position offers a competitive annual salary ranging between $180,000 and $200,000, commensurate with experience and qualifications.
