Key Responsibilities

• Develop comprehensive treatment plans that consider individual patient needs and effectively communicate these plans to residents, caregivers, and other healthcare professionals.
• Order, interpret, and manage laboratory and diagnostic tests, adjusting treatment plans to optimize patient care based on results.
• Utilize our EMR (electronic medical record) system to ensure timely and compliant documentation of all patient interactions and treatment protocols.
• Collaborate with multidisciplinary teams, including local and regional clinical leadership, to implement and achieve institutional care goals that align with value-based care initiatives.
• Deliver exceptional customer service that promotes high levels of patient satisfaction and retention, reinforcing our commitment to quality care.
• Ensure compliance with state and federal regulations, maintaining ethical practices in accordance with professional standards and board certification.
• Coordinate care effectively with Primary Care Physicians (PCPs) and interdisciplinary teams, including specialists and telemedicine providers, to facilitate holistic patient health management.
• Conduct necessary visits as mandated by regulatory requirements—monthly, quarterly, or annually—and provide acute care as the situation demands.
• Participate actively in family conferences, offering guidance on advanced care planning and education tailored to patient needs.
• Evaluate pharmacy recommendations and ensure appropriate care adjustments based on pharmacist communications to enhance patient safety and efficacy of treatment.
• Maintain a presence in assigned facilities to address resident care needs effectively, ensuring face-to-face interactions that enhance communication and satisfaction.
• Assess, diagnose, and manage a wide range of chronic and acute health conditions, focusing on tailored patient education to empower self-management.

Patient Population / Client Focus

The Nurse Practitioner will predominantly serve senior citizens, often with complex healthcare needs, within skilled nursing facilities, assisted living, and other supportive environments. The population may also include low-income individuals requiring accessible healthcare solutions. Qualifications

Compensation and Benefits

The successful candidate will receive a competitive salary ranging from USD 150,000 to 160,000 annually, reflecting the skills and experience brought to the role.
