This full-time role involves providing essential healthcare services to patients with cardiac conditions, working closely with our skilled cardiologists. Work Environment

This position is based in Tonawanda, New York, within a home care facility that provides personalized care in the comfort of our patients' homes. Following an initial 3-month training period consisting of five 8-hour shifts per week, the schedule will transition to four 10-hour shifts, enhancing work-life balance.

Key Responsibilities

• Conduct comprehensive health assessments by obtaining detailed patient histories and performing thorough physical examinations.
• Order and interpret diagnostic tests pertinent to patients' cardiovascular health, utilizing evidence-based practices to inform treatment decisions.
• Develop and prescribe individualized pharmacological and non-pharmacological treatment plans tailored to patients' specific needs.
• Document patient care meticulously, including writing orders and progress notes to reflect updates in treatment plans and patient outcomes.
• Collaborate actively with supervising physicians, providing follow-up care aligned with established protocols, enhancing overall treatment efficacy.
• Engage in occasional hospital rounding to ensure continuity of care for patients requiring inpatient management.
• Communicate effectively with patients and their families, educating them on treatment options and care plans to foster informed decision-making and engagement in their health journey.

Qualifications

• A Master's degree in Nursing from an accredited program is required to ensure comprehensive clinical knowledge.
• Current licensure as a Registered Nurse (RN) in the State of New York is mandatory.
• A minimum of 3 years of clinical experience, with a focus in cardiology being particularly advantageous.
• Basic Life Support (BLS) Certification to ensure preparedness for emergency situations.
• Exceptional interpersonal and communication skills that enable you to work effectively with patients from diverse backgrounds.

Preferred Experience

• Experience in a cardiology setting is preferred but not required, as we are committed to providing adequate training and mentorship for the right candidate.
• Proficiency in electronic health records (EHR) systems to efficiently manage patient information and ensure compliance with healthcare regulations.

Compensation and Benefits

We offer a competitive salary ranging from $150,000 to $160,000 per year, commensurate with experience and qualifications.
