Work Environment

You will work onsite in the comfort of patients’ homes, facilitating a supportive environment that encourages healing and recovery. Given the nature of home care, you will travel to various locations within North Tonawanda and surrounding areas, interacting with patients in diverse communities, thereby enhancing your clinical and communication skills in unique settings.

Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing physical examinations to identify health concerns.
• Order, perform, and interpret diagnostic tests relevant to cardiology, such as EKGs and lab tests, ensuring informed treatment decisions.
• Develop and prescribe individualized treatment plans, incorporating both pharmacological and non-pharmacological interventions tailored to each patient's needs.
• Maintain accurate patient charts, documenting evaluations, treatments, and progress notes diligently to facilitate continuity of care.
• Collaborate with supervising Cardiologists in patient care, conducting regular follow-ups and hospital rounds as required.
• Engage effectively with patients and their families, providing clear communication regarding care plans, education on health maintenance, and emotional support throughout their treatment journey.

Patient Population / Client Focus

This role will focus on adult patients with varying degrees of cardiovascular conditions, ranging from those in the early stages of heart disease to individuals requiring comprehensive management strategies. As you provide care, you will develop lasting relationships with your patients, ensuring not only their physical health but also their emotional and psychological well-being.

Qualifications

• A Master’s degree in Nursing from an accredited academic institution.
• Current licensure as a Registered Nurse (RN) in the State of New York.
• A minimum of 3 years of clinical experience in a relevant healthcare setting, preferably with exposure to cardiology.
• Basic Life Support (BLS) certification is mandatory.
• Exceptional interpersonal skills and a demonstrated ability to build rapport with a diverse patient population.

Preferred Experience

• Familiarity with electronic health record (EHR) systems to manage patient information efficiently.
• Experience working in a home care environment or directly with cardiology patients is highly desirable.
• Ability to lead a multidisciplinary team in collaborative care initiatives that improve patient outcomes.

Compensation and Benefits
