Work Environment

This full-time position takes place in an onsite setting, specifically through home care visits, allowing you to provide personalized and effective healthcare in the comfort of our patients' homes. Key Responsibilities

• Conduct comprehensive health assessments for patients in their homes, identifying acute and chronic medical conditions and formulating effective care plans.
• Manage and diagnose a range of cardiac conditions, employing evidence-based methodologies to ensure optimal patient outcomes.
• Collaborate closely with interdisciplinary teams, including physicians and rehabilitation specialists, to coordinate referrals and ensure seamless follow-up care.

Patient Population / Client Focus

Qualifications

• A Master's degree in Nursing from an accredited institution.
• An active and valid nursing license in the state of New York.
• Current Nurse Practitioner certification with specialized training in cardiology.
• A minimum of 3 years' clinical experience, with a preference for candidates who have worked within community health or public health settings.

Compensation and Benefits
