This position is based onsite, offering home care services that allow our Nurse Practitioners to meet patients in the comfort of their own homes. Key Responsibilities

• Conduct comprehensive patient assessments, including detailed health histories and thorough physical examinations to establish a baseline for treatment.
• Order and interpret diagnostic tests such as echocardiograms, stress tests, and lab work to guide patient management effectively.
• Develop, implement, and revise patient care plans based on clinical findings, collaborating with other healthcare professionals to ensure continuity of care.
• Prescribe appropriate pharmacological and non-pharmacological therapies while adhering to established guidelines for cardiology treatment.
• Educate patients and their families about cardiovascular health, including managing chronic conditions, recognizing warning signs, and implementing lifestyle modifications to promote heart health.
• Actively participate in case discussions, contributing insights from patient interactions to the broader healthcare team for optimal patient care management.
• Maintain accurate and timely documentation of patient encounters, interventions, and treatment plans in compliance with regulatory standards.
• Engage in quality improvement initiatives and stay abreast of the latest research and trends in cardiology practices, ensuring that care delivery remains evidence-based.

Patient Population / Client Focus

This role primarily focuses on adult patients suffering from cardiovascular diseases such as hypertension, heart failure, arrhythmias, and post-operative follow-ups. You will be responsible for managing a diverse patient population, taking into consideration their unique psychosocial backgrounds, treatment barriers, and cultural contexts.

Qualifications

• A Master’s Degree in Nursing, with a specialization in Cardiovascular Health or related field.
• An active Nurse Practitioner license in New York State, with certification in Adult or Family Nurse Practitioner credentials preferred.
• A minimum of 2 years of clinical experience as a Nurse Practitioner, particularly in cardiology or a closely related specialty.
• Strong critical thinking and analytical skills, allowing for comprehensive assessments and appropriate clinical judgments.
• Exceptional communication abilities, both verbal and written, for effective interactions with patients, families, and multidisciplinary teams.

Preferred Experience

• Experience in home healthcare settings is highly desirable.

Compensation and Benefits
