The Primary Care Center Inc. is actively seeking a dedicated and skilled Nurse Practitioner to join our team in the Center for Advanced Cardiac Care (CACC) in Buffalo, New York. Work Environment

Our facility operates primarily in a school setting in Buffalo, NY, fostering a supportive and educational atmosphere. The Nurse Practitioner will report directly to the Chief Nurse Practitioner and will operate under the guidance of the Medical Director within the Division of Cardiology. The environment promotes collaboration, ensuring seamless communication among Physicians, fellow Nurse Practitioners, Registered Nurses, and administrative staff to facilitate comprehensive patient care.

Key Responsibilities

• Collaborate with Cardiologists to assess, diagnose, and manage patients' cardiovascular health.
• Administer outpatient services, focusing on areas such as Electrophysiology, Arrhythmia, and Stress Testing.
• Manage chronic conditions including Hypertension and High Cholesterol, while providing guidance and support to patients regarding lifestyle modifications.
• Conduct evaluations of patients presenting with acute conditions like Coronary Artery Disease, Congestive Heart Failure, and Stroke.
• Perform physical examinations, take medical histories, and prescribe necessary medications in accordance with clinical protocols.
• Educate patients and families on disease management and prevention strategies to empower their health choices.
• Coordinate referrals to relevant specialists when advanced or specialized care is necessary.
• Participate in scheduled consultations and continuous supervision from attending Cardiologists to ensure optimal patient safety and care.

Patient Population / Client Focus

The Nurse Practitioner in this role will serve a diverse population, handling patients with varying degrees of cardiovascular conditions. The focus on outpatient care allows for in-depth relationships with patients, fostering trust and ongoing dialogues about their health management. Qualifications

• A Bachelor's Degree in Nursing, with an accredited Nurse Practitioner program completion.
• Master’s or Doctoral degree preferred.
• Current certification as a Nurse Practitioner in the state of New York.
• Active and unrestricted Nurse Practitioner license.
• Knowledge of medical policies, procedures, and regulations relevant to cardiology.

Preferred Experience

While both experienced Nurse Practitioners and recent graduates will be considered, prior exposure to outpatient Cardiology will be advantageous. Familiarity with cardiology conditions and procedures is highly regarded, including experience in managing patients with risk factors or existing cardiac issues.

Compensation and Benefits
