Full-time Nurse Practitioner for Community Health and Concierge Medicine

Position Overview

This full-time position focuses on providing holistic, patient-centered care directly to individuals and families through our unique concierge medicine model and house call services. Work Environment

This role is conducted onsite, predominantly in patient homes, allowing for a more intimate approach to healthcare that is both accessible and convenient for our patients. Your workweek will generally span approximately 40 hours, typically from 9:00 a.m. to 6:00 p.m., with potential for evening clinics and weekend community outreach events. This setting fosters a collaborative atmosphere where you can engage with patients in their daily lives, ensuring continuity and quality of care.

Key Responsibilities

• Conduct thorough health assessments and screenings in patients' homes, creating individualized care plans based on their unique health profiles.
• Diagnose and manage both acute and chronic medical conditions while emphasizing preventive care and health promotion strategies.
• Collaborate with local community organizations and health stakeholders to design and implement educational programs aimed at improving health literacy and promoting public health.
• Provide culturally sensitive care, advocating for patients' rights and navigating barriers to access in the healthcare system.
• Coordinate referrals and follow-up care, ensuring seamless transitions and continuity for patients receiving various services.
• Maintain meticulous medical records to comply with regulations, ensuring that patient information is accurately recorded and safeguarded.
• Participate actively in community outreach events and health fairs to provide screenings and health education, fostering community engagement.
• Stay informed about the latest community health trends, evidence-based practices, and relevant healthcare policies to enhance service delivery and patient outcomes.

Patient Population / Client Focus

As a Nurse Practitioner, you will serve a diverse patient population in Buffalo, ranging from families to individuals with varying health needs. This includes providing care in underserved areas, addressing chronic health conditions, and fulfilling the preventive health needs of the community. Your role is essential in empowering patients to take charge of their health through education and support.

Qualifications

• A Bachelor's Degree in Nursing, with a Master’s or Doctoral degree from an accredited program preferred.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Active and unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a valid DEA# and NPI# is required.
• Strong clinical assessment and diagnostic abilities, with a commitment to providing high-quality patient care.
• Exceptional communication skills to engage effectively with diverse populations.
• Aptitude for working both independently and collaboratively within a team.
• Proficiency in electronic health records (EHR) systems for managing patient documentation.

Preferred Experience

• A minimum of 3-5 years of clinical experience, particularly in primary care or family medicine settings.
• Previous experience in community health or public health initiatives is highly desirable.

Compensation and Benefits

This position offers a competitive annual salary ranging from $150,000 to $160,000, reflecting your experience and qualifications. Additionally, we provide a sign-on bonus of up to $10,000 for qualified candidates.
