Work Environment

This role is based onsite at our state-of-the-art hospital facility in Tonawanda, New York, where you will work collaboratively with other healthcare professionals in a dynamic and supportive environment. Key Responsibilities

• Conduct comprehensive health assessments, including physical exams and screening tests, to gather information about patient conditions and health practices.
• Diagnose and manage acute and chronic medical conditions, providing appropriate treatment plans and health promotion strategies.
• Collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to develop personalized care plans for patients.
• Advocate for patients by ensuring they have access to necessary health resources and services, while maintaining a culturally sensitive approach.
• Provide education to patients and their families regarding health maintenance, disease prevention, and lifestyle modifications.
• Maintain organized and accurate medical records in compliance with HIPAA and other regulatory standards, utilizing our electronic health record system effectively.
• Participate in community outreach initiatives and health fairs to promote health awareness and screenings within the Tonawanda community.
• Stay informed about the latest healthcare trends and evidence-based practices to enhance patient care delivery.

Patient Population / Client Focus

You will work primarily with diverse patient populations in Tonawanda, including families and individuals with varying healthcare needs, ranging from preventative care to chronic disease management. This position allows you to build long-lasting relationships with patients, empower them through education, and significantly impact their health outcomes.

Qualifications

• A Bachelor’s degree in Nursing (BSN) from an accredited institution.
• Current New York State license as a Nurse Practitioner (APRN), with the ability to practice independently.
• DEA# and NPI# are required to prescribe medications and manage treatment effectively.
• Board certification as a Family Nurse Practitioner (FNP), or Adult-Gerontology Nurse Practitioner (ANP)/Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from an accredited body is mandatory.
• Minimum of 3 years of relevant clinical experience in a healthcare setting, with a preference for applicants with experience in primary care or family medicine.
• Demonstrated strong clinical assessment, diagnostic, and critical thinking skills with the ability to work autonomously.
• Excellent communication and interpersonal abilities to foster a comforting environment for patients and collaborate with the healthcare team.

Preferred Experience

• Experience working within community health or public health settings.
• Proficiency in utilizing electronic health records (EHR) to enhance patient care delivery and documentation accuracy.
• Participation in health education initiatives or community outreach programs is a plus.
• Ability to adapt to changing healthcare demands and patient needs while maintaining high-quality care.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $160,000 annually, commensurate with your experience and qualifications.
