Work Environment

This role is based onsite within our concierge medicine service, where Nurse Practitioners conduct house calls to deliver care directly in the comfort of patients' homes. Key Responsibilities

• Conduct comprehensive initial patient histories and perform thorough physical examinations to assess health status and determine appropriate diagnostic measures.
• Initiate, document, and actively communicate the plan of care, ensuring all necessary screenings and preparations are completed prior to patient treatment.
• Perform daily patient rounds to monitor progress and adjust treatment plans as needed based on clinical findings.
• Write and manage orders for medications, laboratory work, and necessary diagnostic tests, continually reviewing and interpreting results to inform treatment decisions.
• Engage in regular consultations with attending physicians, residents, nursing staff, and other healthcare providers to enhance the quality of patient care.
• Prepare detailed discharge summaries noting prescriptions, follow-up care, and referrals while ensuring patients receive comprehensive post-discharge instructions.
• Participate in performance improvement initiatives, contributing insights to research and educational activities that enhance nursing practice and patient care.

• Current Basic Life Support (BLS) certification is essential.

Compensation and Benefits
