Work Environment

This position requires working onsite in patients' residences, creating a unique and engaging environment where you can build lasting relationships with patients. Unlike typical clinical settings, our concierge model allows for greater flexibility and time with each patient, ensuring that you can provide comprehensive care without the constraints of a traditional office visit. You will collaborate closely with other healthcare professionals to coordinate care, ensuring seamless communication and excellent patient outcomes.

Key Responsibilities

• Conduct and document thorough surgical, medical, and psychosocial health histories and examinations, ensuring all pertinent positive and negative findings are accurately recorded.
• Order and interpret a wide range of diagnostic studies including laboratory tests and imaging, using the results to form differential diagnoses.
• Facilitate patient transitions by coordinating hospital admissions, transfers, and discharges, while ensuring all patients receive appropriate inpatient and outpatient support.
• Develop and implement patient care plans, which include teaching, counseling, and discharge planning, and effectively delegate tasks to other healthcare team members as necessary.
• Perform phone triage for managing acute clinical issues and conduct assessments via telephone according to established protocols, prescribing treatment as indicated.
• Actively participate in the ongoing development and evaluation of care standards that align with institutional policies and regulatory requirements.
• Engage in continuous professional development and education to stay updated with best practices in patient care and apply these within your practice.

Qualifications

• Current New York State Registered Nurse (RN) License.
• National Nurse Practitioner Board Certification (FNP or PNP).
• Valid DEA license for prescribing medications.
• Current Basic Life Support (BCLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
• Proficient in computer systems; ability to quickly adapt to new software applications and electronic health record systems.
• Strong organizational skills and attention to detail are essential for managing patient records and care plans effectively.
• Active New York State Nurse Practitioner License, with the required maintenance and renewal.

Preferred Experience

While not mandatory, experience in a concierge medicine setting or home healthcare environment is highly desirable. Familiarity with telemedicine practices and the ability to handle complex patient cases would be advantageous.

Compensation and Benefits
