Work Environment

This position is entirely onsite as you engage directly with patients in their homes throughout Lancaster and the surrounding areas. By delivering house calls, you will be equipped with the tools you need to assess and treat patients in a familiar setting. Your day-to-day activities will revolve around building strong relationships with patients, thus enhancing their overall healthcare experience.

Key Responsibilities

• Conduct comprehensive health assessments and thorough physical examinations of patients in their homes, ensuring that all findings are meticulously documented.
• Develop, implement, and evaluate individualized nursing care plans tailored to meet the unique needs of each patient.
• Utilize evidence-based practices to deliver patient education and support, helping patients understand their health conditions and management strategies.
• Collaborate closely with multidisciplinary health professionals to review and discuss patient responses to interventions, ensuring continuity and quality of care.
• Maintain accurate and up-to-date documentation of all patient interactions, including progress notes and nursing care plans, to support effective communication among the healthcare team.
• Stay current on medical best practices and advancements in patient care by participating in continuing education and relevant training opportunities.
• Support the operational framework by engaging in performance evaluations and contributing to the enhancement of medical procedures and protocols.

Patient Population / Client Focus

The patients you will serve encompass a broad spectrum of ages and health conditions, including chronic diseases, acute illness, and preventive care. You will focus on developing long-term relationships with your patients, understanding their unique backgrounds and health experiences while striving to empower them in managing their health effectively.

Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
• Current national certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, unrestricted nurse practitioner license in the state of New York is mandatory.
• Legal authorization to work in the United States.
• Exceptional communication skills and the ability to build rapport with diverse patient groups.
• Willingness and capacity to adapt to flexible work schedules to meet patient needs.

Preferred Experience

• Previous experience in a primary care setting or home health care is advantageous.
• Experience with electronic health records (EHR) systems to facilitate proper documentation and care management.
• Knowledge of insurance processes and patient care coordination is a plus.

Compensation and Benefits

The Nurse Practitioner role offers a competitive salary ranging from USD 150,000 to USD 160,000 annually, commensurate with your experience and caseload. We provide a comprehensive benefits package that includes:
