Senior Nurse Practitioner in Concierge Medicine

Health Partners Wellness Solution, LLC is seeking a dedicated full-time Nurse Practitioner (NP) to provide high-quality healthcare services to our patients in Buffalo, New York. Work Environment

The position is based onsite, primarily in patients' homes, which allows for flexibility in addressing the unique circumstances of each individual. Key Responsibilities

• Deliver patient education regarding chronic disease management, preventive care, and healthy lifestyle choices to promote overall well-being.
• Develop tailored treatment plans based on detailed assessments, and clearly communicate these plans to patients, their families, and interdisciplinary teams.
• Order and interpret laboratory and diagnostic tests, and modify treatment strategies based on findings to ensure optimal patient outcomes.
• Maintain thorough and compliant documentation in our EMR system to support continuity of care and ensure regulatory compliance.
• Collaborate with our clinical leadership and operational teams to align practices with organizational goals, particularly for value-based care initiatives.
• Support and participate in strategic initiatives aimed at increasing patient satisfaction and retention.
• Work closely with primary care physicians and specialists to coordinate comprehensive care for complex patients.
• Conduct monthly, quarterly, and annual visits in accordance with regulatory requirements, addressing acute health concerns as they arise.
• Engage in family conferences to discuss care planning and provide education tailored to the needs of patients and their families.
• Assess, diagnose, and manage both chronic and acute health conditions, ensuring timely and effective interventions.
• Address prescription recommendations or communications from pharmacists regarding patient medications, ensuring optimal therapeutic outcomes.

Patient Population / Client Focus

This role focuses on a vulnerable population, including seniors and low-income individuals who may face barriers to accessing traditional healthcare services. By conducting on-site visits, you will have the opportunity to provide crucial healthcare that considers the social determinants of health, contributing significantly to health equity within the community.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or a completed Nurse Practitioner Certificate Program.
• Current, unrestricted Nurse Practitioner license in New York State.
• National certification through AANP or ANCC.
• Active DEA license allowing for prescribing without restrictions.
• Authorization to work within the United States without limitations.

Preferred Experience

• Experience in geriatric care or managing chronic health conditions.
• Demonstrated ability to work effectively in a diverse and dynamic healthcare environment.
• Previous experience in concierge medicine or similar patient-centered care models.

Compensation and Benefits
