Work Environment

Key Responsibilities

As a Nurse Practitioner, your responsibilities will encompass a variety of clinical tasks essential for the well-being of our residents, including but not limited to:

• Developing individualized treatment plans that address both chronic and acute medical conditions. You will effectively communicate these plans to residents, family members, and other healthcare providers.
• Ordering and interpreting various lab tests and diagnostic imaging to inform treatment adjustments as needed.
• Maintaining meticulous and compliant documentation within our electronic medical record (EMR) software to ensure comprehensive patient care and regulatory compliance.
• Collaborating seamlessly with local operations management and clinical leaders to meet organizational and patient care goals, contributing towards initiatives that enhance value-based care.
• Providing superior customer experiences that reflect our commitment to high satisfaction and retention of residents.
• Practicing in accordance with the ethical standards and guidelines set forth by the Board of Nursing and scope of practice regulations.
• Coordinating care with the interdisciplinary team at the facility, including primary care physicians (PCPs) and telemedicine providers.
• Participating actively in family care conferences, providing education on advanced care planning and addressing pharmacy recommendations regarding residents’ medications.
• Completing all regulatory required visits—monthly, quarterly, or annual—as well as managing acute visits based on resident needs.
• Evaluating patients regularly, utilizing a comprehensive approach to managing their health conditions effectively.

Patient Population / Client Focus

You will primarily serve senior residents living in assisted living facilities, often addressing various health challenges associated with aging. Your enthusiasm for geriatric care and attention to the unique needs of low-income populations will greatly benefit our community. Qualifications

The ideal candidate will possess the following qualifications:

• National certification as a Nurse Practitioner (AANP or ANCC).
• An active, unrestricted state license to practice as a Nurse Practitioner.
• Current DEA license for prescribing controlled substances.
• Eligibility to work in the United States without restrictions.
• A degree of Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• Demonstrated passion and commitment to serving senior and low-income populations, along with a flexible work approach.

Preferred Experience

Preference will be given to candidates with prior experience in working within assisted living, skilled nursing facilities, or focused on geriatric healthcare, as well as those familiar with electronic medical record systems that promote efficient and effective patient care management.

Compensation and Benefits
