Work Environment

This position is primarily onsite, providing care directly within the homes of our patients. You will work closely with other healthcare professionals, including nurses, social workers, and therapy staff, to create a supportive and effective care environment. We emphasize efficiency and minimal paperwork, allowing our team members to focus more on patient interactions and care delivery.

Key Responsibilities

• Facilitate seamless hospital admissions, transfers, and discharges, ensuring appropriate outpatient and home care accommodations as needed.
• Coordinate patient care activities based on established priorities, including teaching, counseling, and discharge planning, while directing and delegating tasks to healthcare team members.
• Conduct phone triage for acute clinical issues, performing assessments per established protocols, and prescribing necessary treatments appropriately.
• Document comprehensive surgical, medical, and psychosocial histories, ensuring that all pertinent information is captured for ongoing management.
• Order and interpret diagnostic studies, including laboratory and imaging tests, and synthesize data to inform differential diagnoses.
• Assist in developing, implementing, and evaluating standards of care, fostering continual improvement in professional practice in alignment with regulatory agency requirements.
• Engage proactively in ongoing patient follow-ups and care evaluations to enhance treatment outcomes and patient satisfaction.

Patient Population / Client Focus

Our diverse client population includes individuals of all ages and socioeconomic backgrounds who require home care services. You will primarily serve patients with chronic illnesses, post-operative recovery needs, and acute medical conditions, making it imperative to possess a deep understanding of various disease processes and treatment options.

Qualifications

• Current New York State Nurse Practitioner License.
• NP Board Certification (Family Nurse Practitioner – FNP, or Pediatric Nurse Practitioner – PNP).
• Valid DEA License, BCLS, and ACLS certification.
• Strong organizational skills with keen attention to detail and the ability to perform effectively in a fast-paced environment.
• Proficient in using electronic health record (EHR) systems and able to quickly adapt to new software applications.

Preferred Experience

• Experience in home health care or community-based clinical practice is highly desirable.
• Excellent interpersonal and communication skills, capable of establishing rapport with patients and their families.
• Ability to work collaboratively within a multidisciplinary team to achieve optimal patient outcomes.

Compensation and Benefits
