Position Overview

The US Veterans Health Care Clinic is seeking a dedicated and passionate Community Health Nurse Practitioner to join our team in Buffalo, New York. Key Responsibilities

• Collaborate with community organizations and stakeholders to develop and implement tailored health education programs that empower patients and their families.
• Deliver comprehensive and culturally sensitive healthcare services to individuals residing in the assisted living facility, focusing on preventive care and health promotion.
• Advocate for patient rights and equitable access to healthcare services, addressing barriers to care that may affect our unique patient population.
• Maintain accurate and up-to-date medical records in compliance with HIPAA and regulatory standards, ensuring clear communication within the multidisciplinary team.
• Engage in community outreach initiatives, including health fairs and screenings, to raise awareness about available health resources and promote disease prevention.
• Stay informed about community health trends, evidence-based practices, and policy changes affecting veteran healthcare to continuously refine service delivery.

Patient Population / Client Focus

This role primarily focuses on serving veterans and their families residing in an assisted living facility. The patient population presents a diverse range of health challenges, including chronic diseases, mental health concerns, and age-related conditions. Strong advocacy and sensitivity to cultural and socioeconomic factors are essential to effectively support this unique demographic.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• DEA# and NPI# are required.
• Strong clinical assessment and diagnostic skills, with the ability to develop and implement comprehensive care plans.
• Excellent interpersonal and communication skills, with a proven ability to work effectively with a diverse patient population.

Preferred Experience

• A minimum of 3-5 years of clinical experience, with a strong preference for candidates who have worked in community health or public health settings.
• Experience in geriatric care or working with veterans is highly recommended.
• Familiarity with community health resources and an understanding of local health trends will greatly enhance your effectiveness in this role.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 per year, depending on experience and qualifications.
