Work Environment

The successful candidate will work onsite at our modern assisted living facility in Clarence, New York, which is designed to provide a supportive, comfortable environment for elderly residents. The facility prioritizes patient-centered care, and you will collaborate closely with a multidisciplinary team to promote well-being and enhance health outcomes. Our typical work schedule ranges from 9:00 a.m. to 6:00 p.m., with occasional late clinics and participation in weekend community outreach events.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents of the assisted living facility, ensuring personalized care plans.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health promotion strategies.
• Collaborate with community partners to design and implement impactful health education programs that address common health issues faced by the elderly.
• Provide culturally sensitive care while advocating for patients' rights and their access to necessary healthcare services.
• Coordinate referrals and follow-up care to ensure continuity and efficacy in treatment plans, addressing any barriers to care.
• Maintain accurate and competent medical records in compliance with all regulatory standards, ensuring confidentiality and data integrity.
• Engage actively in community outreach events and health fairs, delivering health screenings and educating the public on health topics.
• Stay informed on current health trends, community health initiatives, and relevant nursing practices to provide the best care possible.

Patient Population / Client Focus

This role focuses primarily on the elderly population residing in the assisted living facility. You will encounter a diverse range of medical and social needs, from managing chronic conditions to enhancing quality of life. Your interaction with residents will be vital in understanding their personal health challenges and preferences, creating a compassionate care environment that prioritizes their well-being.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP).
• Unrestricted Nurse Practitioner (APRN) license in New York State.
• Current DEA and NPI numbers are required.

Preferred Experience

• A minimum of 3–5 years of clinical experience, particularly in primary care or family medicine settings.
• Demonstrated strong clinical assessment, diagnostic skills, and proficiency in using electronic health record (EHR) systems.
• Outstanding communication and interpersonal skills, with an ability to work effectively with a diverse population of patients and colleagues.

Compensation and Benefits

• Competitive annual salary ranging from $120,000 to $130,000, based on experience and qualifications.
