Key Responsibilities

• Deliver exemplary patient care that aligns with national and community health standards, ensuring adherence to evidence-based practices.
• Collaborate effectively with the office staff to develop efficient office protocols and streamline care processes.
• Serve as a liaison between practice management and the broader hospital administration, advocating for both teams' interests and objectives.
• Engage in patient management and care coordination, actively unifying the goals of the practice with the strategic aims of the health system.
• Conduct thorough assessments of patients’ health statuses, interpreting data to tailor personalized care plans.
• Maintain complete and timely patient documentation, ensuring compliance with all regulatory requirements while responding to relevant stakeholders.

• Attend to administrative responsibilities such as staying current on professional licenses, certifications, and regulatory education to meet employment prerequisites.

Patient Population / Client Focus

The Nurse Practitioner primarily serves an adult population within the internal medicine specialty, addressing a wide variety of health issues ranging from chronic disease management to routine health maintenance. The practitioner will ensure that treatment plans reflect the unique medical histories and personal circumstances of each patient.

Qualifications

• NYS certification and registration as a Family, Adult, or Pediatric Nurse Practitioner is required.
• Current AANP Board Certification and a valid DEA license are essential.
• Ability to obtain necessary privileges at F. F. Thompson Health.
• Proficiency in ICD10 coding is preferred, enhancing case documentation and billing accuracy.

Preferred Experience

While prior experience as a Nurse Practitioner in an internal medicine setting is preferred, candidates with a strong passion for patient care and the willingness to learn will also be considered. A proactive approach to professional development and collaboration is encouraged.
