Work Environment

You will work in a vibrant outpatient office setting, where you will engage with patients in a supportive and collaborative atmosphere. Our office operates primarily from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. As part of our commitment to community engagement, you may be required to participate in outreach events or health fairs during weekends, enhancing your role in promoting community health.

Key Responsibilities

• Conduct thorough health assessments and screenings tailored to the individual and family healthcare needs.
• Diagnose, manage, and treat both acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.
• Develop and implement health education programs in collaboration with community organizations to foster healthy lifestyle choices.
• Advocate for patient rights and equitable healthcare access, providing culturally sensitive care to diverse populations.
• Coordinate referrals and facilitate follow-up care to ensure continuity and comprehensive management of patient health.
• Maintain detailed and accurate medical records, ensuring compliance with regulatory standards and best practices.
• Stay current on community health trends, evidence-based practices, and relevant healthcare policies to inform your practice.
• Engage actively in community outreach initiatives, such as health fairs, to increase awareness and provide essential health screenings.

Patient Population / Client Focus

Your primary focus will be on individuals and families within the local community, particularly those who may face barriers to accessing healthcare. You will work with a diverse patient population that includes individuals from various socioeconomic backgrounds, cultural orientations, and age groups. By understanding and responding to the unique challenges faced by these communities, you will play a pivotal role in shaping their health outcomes positively.

Qualifications

• Current certification as a Nurse Practitioner (FNP) and active state nursing license in New York.
• A Master's degree in nursing from an accredited institution.

Preferred Experience

Compensation and Benefits
