State Health, LLC is seeking a dedicated and skilled Nurse Practitioner specializing in Internal Medicine to join our team in East Amherst, New York. Work Environment

This role is caretaker-centric, with work primarily conducted in the patient's home and if needed, during clinic hours. You will be a vital member of a collaborative care team consisting of a physician, fellow nurse practitioners or physician assistants, medical assistants, and support staff, including dedicated medical scribes and care managers. Key Responsibilities

• Provide exceptional primary care services, ensuring that patients receive comprehensive health assessments and diagnosis.
• Conduct routine office visits for both acute and non-acute health concerns, using evidence-based practices to guide treatment decisions.
• Administer annual wellness visits and health risk assessments, encompassing holistic evaluations that emphasize thorough documentation.
• Facilitate care coordination with various healthcare professionals, specialists, testing facilities, and community agencies to ensure seamless patient care.
• Engage in population health leadership initiatives, such as ensuring compliance with evidence-based screenings, including breast cancer exams every two years for eligible patients.
• Assist the care team with phone triage, promptly addressing patient inquiries and facilitating necessary follow-ups.
• Provide educational resources to patients regarding their health conditions, treatment options, and personalized care plans.
• Conduct home visits when necessary, ensuring accessibility to quality healthcare in the patient’s environment.
• Report directly to the Medical Director while collaborating closely with operational leadership for strategic initiatives.

Patient Population / Client Focus

Your primary focus will be on older adults who are on Medicare, a demographic that often navigates complex health issues requiring dedicated care and attention. Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner certificate program.
• National board certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from recognized certifying bodies like AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• A strong foundation in primary care, with experience particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with Hierarchical Condition Categories (HCC) documentation, ICD-10 coding, and Health Risk Assessments (HRA).
• A passion for collaborative teamwork, with a strong desire to contribute to an innovative healthcare model focusing on population health.
• Bilingual proficiency is an asset in our diverse community.

Preferred Experience

While experience in geriatric care or internal medicine is highly valued, candidates with a general background in family medicine will also be considered. Comfort in navigating various electronic health records systems and health care technologies is preferred to enhance operational efficiencies. Compensation and Benefits

The compensation for the Nurse Practitioner position ranges from USD 115,000 to 150,000 annually, with the final salary commensurate with experience and expertise. In addition to a competitive salary, we offer a comprehensive benefits package that includes:
