Work Environment

This role is primarily onsite, allowing you to engage directly with patients in their homes or community settings. We offer a collaborative atmosphere where healthcare providers work closely with each other and various community stakeholders. You will have a flexible schedule with typical hours ranging from 9:00 a.m. to 6:00 p.m., some evening clinics until 8:00 p.m., and potential weekend engagements for community outreach events. With an average workweek of approximately 40 hours, we aim to promote both staff wellbeing and optimal work-life balance.

Key Responsibilities

• Conduct thorough health assessments and screenings for diverse individuals and families to identify healthcare needs and challenges.
• Diagnose, treat, and manage a wide range of acute and chronic medical conditions, providing tailored preventive care and health promotion practices.
• Develop and implement community health education programs by collaborating with local organizations and stakeholders to address pressing health concerns.
• Advocate for culturally sensitive healthcare practices, ensuring that all patients feel valued and respected.
• Coordinate referrals and facilitate follow-up care to maintain continuity and quality of patient care.
• Maintain meticulous medical records in compliance with legal and regulatory standards, utilizing electronic health records (EHR) systems effectively.
• Participate in community outreach activities, such as health fairs, to enhance awareness and provide essential screenings.
• Stay informed about community health trends and emerging evidence-based practices to continuously improve the care delivered.

Patient Population / Client Focus

As a Nurse Practitioner at the Sanctuary of Clarity, you will interact with a diverse patient population, including individuals from various socioeconomic backgrounds and cultural identities. Your ability to connect with patients, understand their unique needs, and tailor healthcare solutions will be crucial in fostering trustful relationships and optimizing their health outcomes.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program is required.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid DEA# and NPI# are mandatory.
• Proficient in using electronic health records (EHR) systems for patient documentation and communication.

Preferred Experience

• 3-5 years of clinical experience, especially in primary care, family medicine, community health, or public health settings.
• Strong clinical assessment and diagnostic skills, along with excellent communication and interpersonal abilities.
• An aptitude for collaborating effectively with a multidisciplinary team and diverse patient populations.

Compensation and Benefits

The Nurse Practitioner position offers an attractive annual salary ranging from USD 150,000 to 160,000, commensurate with experience and qualifications. In addition, we offer a sign-on bonus of up to $10,000.
