Key Responsibilities

• Collaborate with a diverse team of healthcare providers to ensure continuity of care across the healthcare spectrum for assigned patients.
• Perform in-depth assessments by obtaining medical, physical, and psycho-social history, along with conducting comprehensive health evaluations tailored to each patient’s age and background.
• Implement preventive screening procedures through non-invasive methods appropriate to the patient's demographics.
• Formulate differential diagnoses based on thorough review of patient history and clinical findings.
• Design and deliver targeted patient education plans that address individual health literacy needs and promote self-management of conditions.
• Prescribe and manage pharmacologic and non-pharmacologic therapies grounded in the latest medical evidence and practices.
• Utilize electronic health record (EHR) systems for documentation and care coordination, ensuring efficient management of patient information.

Patient Population / Client Focus

This role primarily focuses on adult patients within the context of primary and urgent care. As a Nurse Practitioner, you will address a variety of health conditions, ranging from chronic diseases to acute health issues, ensuring each patient receives personalized and attentive care. Qualifications

• Master’s Degree in Nursing with a specialization as a Nurse Practitioner.
• Current and valid Registered Nurse (RN) license in the state of New York.
• At least 3 years of relevant clinical experience in a primary care setting, with a focus on Internal Medicine.
• Strong understanding of medical technology and proficiency with electronic health records (EHR) systems.
