Work Environment

The Nurse Practitioner will work collaboratively within a dynamic care team composed of physicians, fellow nurse practitioners or physician assistants, medical assistants, and support staff including registered nurses and medical scribes. Key Responsibilities

• Deliver high-quality primary care and clinical management to adult patients, focusing on both routine and acute healthcare needs.
• Conduct thorough office visits, performing assessments, diagnosing conditions, and developing treatment plans tailored to individual patients.
• Administer annual wellness visits and health risk assessments, incorporating a holistic approach that emphasizes thorough documentation for continuity of care.
• Facilitate care coordination with specialists, testing facilities, and community agencies to ensure comprehensive patient support.
• Lead initiatives around population health management, ensuring all patients receive appropriate screenings and preventive care based on established guidelines.
• Engage in phone triage, patient outreach, and education, empowering patients to understand their health conditions, treatment options, and care plans.
• Conduct home visits as required, providing care in the patients' environment to better meet their needs.
• Participate in marketing activities and community health programs to promote our services and educate the community.
• Report directly to the Medical Director and work closely with the operational leadership team, contributing to the overall strategic goals of the organization.

Patient Population / Client Focus

You will primarily serve older adults on Medicare, focusing on maintaining their health and quality of life. Our patients often have multi-faceted healthcare needs and complex conditions, requiring a tailored approach to care that encourages long-term engagement and trust. By fostering close relationships with your patients, you will play a crucial role in guiding them through their healthcare journeys.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through either AANP or ANCC.
• Possession of an active, non-probationary Nurse Practitioner license in the state of New York.
• Current DEA license and familiarity with state and federal regulatory requirements.
• A strong foundation in primary care settings, with experience in internal medicine, geriatrics, or family medicine.
• Understanding of documentation practices, specifically HCC (Hierarchical Condition Categories), ICD-10 coding, and health risk assessments.
• A demonstrated passion for collaboration, teamwork, and comprehensive patient care.

Preferred Experience

• Experience in concierge medicine or similar patient-centric practice models.
• Proficiency in using electronic medical records (EMR) systems and clinical documentation tools.
• Ability to adapt to a fast-paced environment while maintaining attention to detail and high standards of care.
• Previous involvement in community health initiatives or outreach programs.

Compensation and Benefits

The salary range for this position is between $120,000 and $160,000 annually, commensurate with experience and credentials.
