Full-Time Nurse Practitioner - Concierge Medicine

Work Environment

This position is primarily onsite, allowing for direct patient interaction and engagement. You will collaborate within a multidisciplinary care team that includes physicians, fellow nurse practitioners, physician assistants, medical assistants, and dedicated support staff such as medical scribes and care managers. Key Responsibilities

• Provide exceptional primary care services to patients, addressing both routine and acute healthcare needs.
• Conduct thorough office visits and perform annual wellness assessments, utilizing a holistic view of patient health.
• Facilitate care coordination with other healthcare providers, specialists, testing labs, and community resources to support patient needs.
• Lead initiatives in population health management by ensuring patients receive routine screenings, such as biannual breast cancer screenings for eligible individuals.
• Engage in phone triage and outreach, assisting patients with scheduling, follow-ups, and various health-related inquiries.
• Educate patients about their health conditions, treatment options, and care plans to foster better understanding and adherence.
• Participate in community outreach and promotional activities to enhance public awareness of our services.
• Conduct home visits as needed to provide care for patients with limited mobility or accessibility issues.
• Report directly to the Medical Director, maintaining close collaboration with operational leadership for effective practice management.

Patient Population / Client Focus

Your primary focus will be on adult patients, particularly older adults enrolled in Medicare. You will play a key role in ensuring that this demographic receives personalized care, enhancing their quality of life through preventive health measures and chronic disease management. By working within the community, you will have the opportunity to make a meaningful impact on the health outcomes of this population.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from recognized bodies such as AANP or ANCC.
• Possession of an active and non-probationary state Nurse Practitioner license.
• Active DEA license for prescribing medications.
• Proven experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine disciplines.
• Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is highly desirable.
• Demonstrated ability to work effectively as part of a team within a collaborative care environment.

Preferred Experience

• Experience conducting home calls or in concierge medical settings is a plus.
• Familiarity with electronic health record (EHR) systems and patient management software.
• Strong communication skills that foster patient education and engagement.

Compensation and Benefits

This position offers a competitive annual salary ranging from USD 120,000 to 160,000, commensurate with experience. In addition, we provide a comprehensive benefits package, including health insurance, retirement savings options, paid time off, and ongoing professional development opportunities.
