Key Responsibilities

• Conduct comprehensive health assessments for patients, including detailed medical histories and physical examinations.
• Diagnose and manage acute and chronic health issues, developing tailored treatment plans in line with individual patient needs.
• Promote health education and preventive care strategies to empower patients in managing their health effectively.
• Formulate and prescribe medications, ordering and interpreting diagnostic tests as necessary to monitor patient progress.
• Maintain accurate and up-to-date patient records using our electronic health record (EHR) system, following all privacy regulations.
• Collaborate effectively with a multi-disciplinary team to ensure a holistic approach to patient care.
• Participate in community outreach activities, promoting health awareness and education among underserved populations.

Patient Population / Client Focus

You will primarily serve a diverse range of patients, including families and individuals seeking comprehensive, personalized medical care. Our concierge medicine model allows us to provide extended time with each patient to address their specific health concerns and preferences. Emphasizing a strong doctor-patient relationship, your role will bridge the gap between available healthcare options and the needs of the community, particularly for those facing barriers to traditional medical care.

Qualifications

• A Bachelor’s Degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner.
• An active New York State Nursing License.
• A minimum of three years of clinical experience, with a strong preference for experience in community health or public health settings.
• Exceptional interpersonal and communication skills to connect with diverse populations effectively.
• A commitment to social justice, health equity, and advocacy for vulnerable communities.

Preferred Experience

• Experience within a concierge or house call medical practice.

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
