Key Responsibilities\n

• Utilize your expertise in social work to conduct comprehensive psychosocial assessments and formulate independent diagnoses that encompass the emotional, psychological, and social complexities faced by patients.

• Collaborate closely with patients to co-create tailored treatment plans that reflect their unique circumstances and values, ensuring that their voices are central in the care process.

• Employ evidence-based therapeutic techniques, such as motivational interviewing and solution-focused therapy, to deliver effective behavioral interventions designed to improve wellbeing and cope with life transitions.

• Work alongside a diverse team of clinicians, including psychologists and medical providers, to coordinate comprehensive care that is focused on the whole person, thus enhancing treatment efficacy.

• Monitor and document patient progress meticulously, utilizing both in-person and virtual consultations to ensure continued alignment with treatment objectives.

Patient Population / Client Focus\n

This role primarily serves patients at the hospice center, who may be grappling with terminal illnesses and complex emotional responses to their conditions. You will also engage with families, providing them with necessary emotional support and guidance.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, with an emphasis on clinical training.

• A current, valid license to practice as a Licensed Clinical Social Worker (LCSW) in California, with no restrictions on your practice.

• At least one year of post-licensure experience working independently as a therapist, preferably in mental health or hospice settings.

Compensation and Benefits\n
