Key Responsibilities\n

• Initiate comprehensive discharge planning from the moment of patient admission, ensuring smooth transitions and preparedness for post-discharge life.
\n
• Conduct in-depth discharge sessions to reinforce coping skills, clarify transition plans, and confirm that patients and their families understand the resources available to them.
\n
• Collaborate with patients and their loved ones to confirm crucial follow-up appointments before discharge, supporting continuity of care.
\n
• Engage with referral sources and outpatient providers to maintain continuity of care at each stage—admission, mid-point, and discharge.
\n
• Facilitate patient transfers to higher levels of care when necessary, ensuring a safe and supportive transition.
\n
• Participate in outreach initiatives and program tours to educate the community about our services.
\n
• Manage crises effectively during therapy sessions and within the care environment, providing immediate support and intervention.
\n
• Offer guidance and support to milieu staff in their engagements with patients and family members, fostering a therapeutic atmosphere.
\n
• Work cohesively with multidisciplinary teams to perpetually elevate the quality of clinical services delivered.
\n
• Conduct thorough assessments to evaluate patients for potential suicidality and risk factors affecting self or others.
\n
• Extend clinical support to team members during absences, ensuring uninterrupted patient care.
\n
• Collaborate with Utilization Review and Business Departments regarding authorization processes and payment structures.

Patient Population / Client Focus\n

The ideal candidate will work primarily with patients undergoing hospice care, focusing on individuals facing end-of-life challenges. Qualifications\n

• Master’s degree in Social Work, Counseling, Psychology, or a related mental health discipline.

• Must hold an independent license as a Licensed Clinical Social Worker (LCSW) in California.

• Demonstrated capability in completing and submitting high-quality documentation of services, including initial assessments, SMART treatment plans, and clinical records.

• Familiarity and competency with Office 365 and Electronic Health Record (EHR) systems.
