This position is primarily remote, allowing you to work flexibly from home while being part of a cohesive outpatient team. Key Responsibilities\n

As an LPC at Big Leap Family Guidance Centers, you will be responsible for:

• Conducting thorough patient intakes that include diagnostic and functional evaluations, making informed recommendations regarding treatment goals and plans to primary care providers.

• Delivering brief, focused interventions for patients requiring mental health services, and advising providers on appropriate referrals to specialty mental health facilities when necessary.
\n
• Monitoring patient progress, developing relapse prevention plans, and assisting clients in maintaining stable functioning.
\n
• Identifying at-risk patients and collaborating with them to create protective strategies that can help avert worsening mental health conditions.
\n
• Coordinating health services in relation to behavioral health care, ensuring all treatment providers are informed and collaborating effectively.
\n
• Maintaining accurate clinical records and documentation that track patient progress, treatment plans, and outcomes.
\n
• Providing consultation on behavioral health services to clinic management and other team members, contributing to program evaluations and improvements.

Qualifications\n

The ideal candidate will have:

• A Master's Degree in Clinical Counseling from an accredited institution.

• A valid Licensed Professional Counselor (LPC) license.

• Comprehensive knowledge of mental health assessment, treatment planning, behavioral therapy methods, and developmental psychopathology.

• The ability to develop effective therapeutic relationships with patients and their families, maintaining a client-centered approach.

• Excellent communication skills, allowing for clear and compassionate interactions with clients and interdisciplinary team members.

Preferred Experience\n

While not mandatory, candidates who possess the following will be given preference:

• Bi-lingual proficiency in English and Spanish to better communicate with our diverse patient base.

• Experience working with Federally Qualified Health Centers (FQHC).

• Familiarity with electronic health records (EHR) systems and telehealth practices.
