We're seeking an experienced Director of Services & Care Transitions (RN) to join our executive leadership team and help shape the future of innovative, community-based healthcare. This role is ideal for a mission-driven nursing leader who is passionate about improving outcomes for vulnerable populations while building high-performing clinical teams and scalable care delivery programs. Reporting directly to the CEO, you'll provide strategic leadership for Illumination Health's clinical operations, overseeing Recuperative Care and Enhanced Care Management (ECM) programs while partnering across the organization to expand integrated, person-centered services. You'll lead talented clinical professionals, collaborate with operational leadership, and help drive innovative programs that improve care transitions, reduce barriers to healthcare, and strengthen our impact throughout the communities we serve. Responsibilities Lead and mentor multidisciplinary clinical teams, including RNs, LVNs, and Medical Assistants Oversee Recuperative Care and Enhanced Care Management (ECM) programs Develop and scale integrated care management models serving medically and socially complex populations Drive quality improvement initiatives, clinical excellence, and regulatory compliance Partner with operations to improve care coordination, client outcomes, and service delivery Utilize data and performance metrics to strengthen clinical effectiveness Build strategic partnerships with healthcare organizations, payers, and community stakeholders Serve as a key executive leader helping shape organizational strategy and future growth. We're Looking For You are a collaborative healthcare leader who combines strong clinical expertise with a deep commitment to serving underserved communities. Qualifications include: Active California Registered Nurse (RN) license BSN required 5+ years of progressive healthcare leadership experience Experience leading interdisciplinary clinical and care management teams Strong knowledge of ECM, Medi-Cal populations, care coordination, and case management Passion for improving health equity and serving vulnerable populations Experience with Recuperative Care, CalAIM, community-based healthcare, or homeless services is highly valued!
