Joint Ventures Physical Therapy is seeking a dedicated and passionate Physical Therapist to join our outstanding team in Perth Amboy, New Jersey. This full-time position will involve direct, hands-on patient care in a home care setting, focusing on helping individuals regain their mobility, restore motion, and improve their overall quality of life.

Work Environment

You will work primarily in patients' homes, allowing you to deliver personalized care where comfort is paramount. Our home care facility emphasizes flexibility, enabling you to create tailored rehabilitation plans that accommodate each patient's unique needs. This role's regular hours are 9 am to 5 pm, Monday through Friday, providing a consistent schedule to help maintain a healthy work-life balance.

Key Responsibilities

• Evaluate patients' conditions through detailed assessments and diagnostic tests to determine appropriate treatment plans.
• Design and implement individualized treatment programs that address patients' specific goals and requirements, utilizing a variety of therapeutic modalities.
• Maintain a manageable caseload, ensuring each patient receives the time and focus necessary for effective healing.
• Utilize a variety of manual therapy techniques and exercises tailored to enhance patients' motion, strength, and overall functional capabilities.
• Collaborate with other healthcare professionals to ensure comprehensive and integrated patient care.
• Monitor patient progress and adjust treatment plans as needed to align with evolving patient needs and outcomes.
• Provide educational resources and supportive advice to patients and their families to promote adherence to treatment and lifestyle changes.
• Document all patient interactions and treatments accurately in an electronic medical records (EMR) system, specifically Web PT, to maintain compliance and facilitate billing and care coordination.
• Foster a motivating and encouraging environment that inspires patients to take an active role in their recovery journey.

Patient Population / Client Focus

Our patients range from athletes recovering from sports-related injuries to individuals experiencing post-operative rehabilitation needs. Many of our clients also have chronic pain conditions or require therapy to support daily activities. Adapting to each patient's unique challenges and fostering a supportive healing environment is crucial in this role.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field is required; candidates with a Doctorate in Physical Therapy (DPT) will be highly favored.
• An active Physical Therapist license in New Jersey without any history of sanctions or board proceedings.
• Proficient in a variety of manual therapy techniques with proven expertise in orthopedic care.
• Exceptional skills in developing and implementing successful exercise and rehabilitation programs.
• Familiarity with EMR software, particularly Web PT, for efficient documentation and management.
• A strong commitment to continuous professional development and knowledge of the latest evidence-based practices in physical therapy.

Preferred Experience

• Previous experience in outpatient settings, particularly in home care, will be considered a significant advantage.
• Demonstrated ability to implement cost-effective treatment strategies that prioritize patient wellness and functional improvements.
• A background in sports medicine or related rehabilitation fields is desirable.

Compensation and Benefits
